Prior to any Permanent Make-Up (PMU) (microblading, microblade & shade, eyeliner, etc.) scalp micropigmentation, or camouflage work to be performed, you will be required to complete necessary paperwork to ensure I have information necessary to provide you with best service possible and to be sure there are no contra-indications prior to appointments. Forms will be provided to you after your consultation. You'll bring your completed forms to your initial appointment.
